What is Hydrochlorothiazide + Olmesartan Medoxomil used for?
- Treatment of hypertension in adults to lower blood pressure.
- Use as combination therapy when blood pressure is not adequately controlled with a single antihypertensive medicine, or when the prescriber considers combined treatment appropriate.
Dosage & Administration
The dose and strength should be selected by the prescribing doctor based on current blood pressure, previous treatment, kidney function, electrolytes, and other medicines. It is usually taken once daily, with or without food, at the same time each day. Do not change the dose or stop treatment without medical advice.
Side Effects
- Common effects may include dizziness, light-headedness, headache, tiredness, increased urination, nausea, or low blood pressure, especially after starting treatment.
- Hydrochlorothiazide may cause changes in blood salts and body chemistry, such as low potassium, low sodium, increased uric acid, increased blood sugar, or increased cholesterol/triglycerides.
- Serious effects can include fainting, dehydration, kidney problems, severe allergic reactions, worsening gout, eye pain or sudden vision changes, severe skin reactions, or symptoms of high potassium such as muscle weakness or irregular heartbeat.
- Olmesartan has rarely been associated with severe, chronic diarrhoea with weight loss; seek medical advice if this occurs.
Precautions & Warnings
Use with caution in people with kidney disease, liver disease, heart failure, dehydration, low blood pressure, electrolyte imbalance, diabetes, gout, systemic lupus erythematosus, history of angioedema or allergy to sulfonamide-derived medicines, or in older adults. Blood pressure, kidney function, and electrolytes may need monitoring. Use this medicine only under the guidance of a registered doctor or pharmacist.
Drug Interactions
- Lithium: may increase lithium levels and toxicity; combination is generally avoided unless closely monitored.
- NSAIDs such as ibuprofen, naproxen, or diclofenac: may reduce blood pressure effect and increase kidney risk, especially in dehydrated or elderly patients.
- ACE inhibitors, aliskiren, potassium supplements, potassium-sparing diuretics, or salt substitutes containing potassium: may increase the risk of high potassium or kidney problems; aliskiren must not be used with this combination in patients with diabetes.
- Other blood pressure-lowering medicines, alcohol, sedatives, or strong vasodilators: may increase dizziness or low blood pressure.
- Diabetes medicines including insulin or oral antidiabetics: blood sugar control may change and dose adjustment may be needed.
- Corticosteroids, ACTH, amphotericin B, stimulant laxatives, or some diuretics: may increase the risk of low potassium.
- Cholestyramine or colestipol may reduce absorption of hydrochlorothiazide if taken together.
- Digoxin and some antiarrhythmic medicines may have increased risk of effects if potassium or magnesium becomes low.

- Known hypersensitivity to olmesartan, hydrochlorothiazide, sulfonamide-derived medicines, or any component of the product.
- Pregnancy, especially after the first trimester; discontinue as soon as pregnancy is detected unless a doctor advises an alternative plan.
- Anuria, meaning inability to produce urine.
- Concomitant use with aliskiren in patients with diabetes.
- Severe kidney impairment, severe liver impairment, cholestasis, or biliary obstruction, unless a specialist specifically determines otherwise.
Pregnancy & Lactation
This combination should not be used during pregnancy because olmesartan can cause serious injury or death to the unborn baby, particularly in the second and third trimesters. Patients planning pregnancy or who become pregnant should contact a doctor promptly for a safer alternative. Breastfeeding is not generally recommended while using this combination because hydrochlorothiazide can pass into breast milk and may reduce milk production; consult a doctor for an appropriate option.
Storage
Store at room temperature in a dry place, away from excessive heat, moisture, and direct sunlight. Keep in the original packaging and out of the reach of children. Do not use after the expiry date.
