What is Tribcol 5?
Tribcol 5 5 mg tablet by Genvio Pharma Ltd. is a medicine containing Trihexyphenidyl Hydrochloride. In Bangladesh, it is mainly used to help control symptoms of Parkinsonism and certain medicine-induced movement problems, such as tremor, stiffness and abnormal muscle spasms.
Trihexyphenidyl Hydrochloride works by blocking muscarinic acetylcholine receptors in the nervous system, helping to improve the balance of brain chemicals involved in movement. A key caution is that it can cause anticholinergic effects such as blurred vision, dry mouth, constipation, drowsiness, urinary difficulty and worsening of narrow-angle glaucoma.
Indications
- Adjunctive treatment of Parkinsonism, including idiopathic Parkinson disease and other forms of Parkinsonism where an anticholinergic is appropriate.
- Management of extrapyramidal symptoms caused by certain medicines, especially antipsychotic medicines, such as acute dystonia, drug-induced parkinsonism and similar movement symptoms.
- It is not a treatment for tardive dyskinesia and may worsen it in some patients.
Dosage & Administration
Adult dosing is individualized. Treatment is usually started with a low dose and increased gradually according to response and tolerability. It is commonly taken in divided doses, often with food if stomach upset occurs. The exact dose, timing and duration should be decided by the prescribing doctor. Do not stop Trihexyphenidyl Hydrochloride suddenly unless advised, as symptoms may worsen.
Side Effects
- Common: dry mouth, blurred vision, dilated pupils, constipation, nausea, dizziness, drowsiness, nervousness, headache and reduced sweating.
- Urinary difficulty or urinary retention, especially in older men or people with prostate problems.
- Fast heartbeat or palpitations.
- Confusion, agitation, memory problems, hallucinations or mood changes, especially in older adults or at higher doses.
- Eye pain, sudden blurred vision or halos around lights may suggest acute glaucoma and needs urgent medical help.
- Severe constipation, abdominal swelling, high fever, heat intolerance or inability to sweat should be treated as serious warning signs.
- Allergic reactions such as rash, swelling or breathing difficulty are rare but require urgent care.
Precautions & Warnings
Use Trihexyphenidyl Hydrochloride carefully in older adults and in people with glaucoma or raised eye pressure, prostate enlargement, urinary retention, bowel obstruction or severe constipation, heart disease, high blood pressure, liver or kidney impairment, myasthenia gravis, mental health disorders, or a history of substance misuse. It may cause drowsiness, dizziness or blurred vision, so avoid driving or operating machinery until the effect is known. Reduced sweating can increase the risk of heat illness, especially in hot weather. Use this medicine only under the guidance of a registered doctor or pharmacist.
Drug Interactions
- Other anticholinergic medicines, including some antihistamines, tricyclic antidepressants, antipsychotics, antispasmodics and medicines for urinary symptoms, can increase dry mouth, constipation, blurred vision, confusion and urinary retention.
- Alcohol and other sedating medicines can increase drowsiness, dizziness and impaired coordination.
- Some antipsychotic medicines may have increased anticholinergic side effects when used together with Trihexyphenidyl Hydrochloride; movement symptoms should be monitored by a doctor.
- Medicines that increase gastrointestinal motility, such as metoclopramide, may have reduced effect because anticholinergics slow gut movement.
- Because Trihexyphenidyl Hydrochloride can slow stomach emptying, it may affect the absorption or tolerability of some oral medicines; ask a pharmacist before combining it with new medicines.
Contraindications
- Known hypersensitivity to trihexyphenidyl or any component of the product.
- Untreated narrow-angle glaucoma or risk of acute angle-closure glaucoma.
- Use is generally avoided in significant urinary retention, severe bowel obstruction, paralytic ileus or severe gastrointestinal narrowing unless a specialist specifically decides otherwise.
Pregnancy & Lactation
There is limited human safety information for use of Trihexyphenidyl Hydrochloride during pregnancy. It should be used in pregnancy only when the expected benefit justifies the possible risk and when prescribed by a doctor. It is not known whether trihexyphenidyl passes into breast milk; anticholinergic medicines may also reduce milk production. Breastfeeding mothers should use it only after medical advice and should monitor the infant for unusual sleepiness, feeding problems or poor weight gain.
Storage Conditions
Store at room temperature, away from excessive heat, direct sunlight and moisture. Keep the container tightly closed and out of the reach of children. Do not use after the expiry date, and follow the label instructions for any solution or syrup after opening.
