Empagliflozin + Linagliptin

Empagliflozin + Linagliptin is a antidiabetic combination (sglt2 inhibitor + dpp-4 inhibitor) medicine available in Bangladesh under many brand names. Below are its uses, dosage, side effects and precautions — and all brands with their current prices.

• Improvement of blood glucose control in adults with type 2 diabetes mellitus, together with diet and exercise.
• It may be considered when treatment with both an SGLT2 inhibitor and a DPP-4 inhibitor is appropriate, as decided by a doctor.
• It is not indicated for type 1 diabetes mellitus or for treatment of diabetic ketoacidosis.

The usual adult use is as a once-daily oral tablet, commonly taken in the morning, with or without food. The exact strength, starting dose, and any dose change should be decided by the prescribing doctor based on current diabetes medicines, kidney function, blood glucose targets, and tolerance. Do not start, stop, or double the dose without medical advice.

• Common effects may include increased urination, thirst, urinary tract infection, genital fungal infection, runny or blocked nose, sore throat, and mild stomach upset.
• Low blood sugar may occur, especially when used with insulin or a sulfonylurea.
• Dehydration, dizziness, low blood pressure, or worsening kidney function can occur, particularly in older adults, people taking diuretics, or those with poor fluid intake.
• Serious but uncommon effects include diabetic ketoacidosis, severe urinary tract infection, Fournier’s gangrene, pancreatitis, severe allergic reaction or angioedema, severe joint pain, and bullous pemphigoid.

Use caution in people with kidney disease, liver problems, heart failure with low blood pressure risk, recurrent urinary or genital infections, history of pancreatitis, dehydration, heavy alcohol use, planned surgery, prolonged fasting, or acute serious illness. Kidney function and hydration status should be checked as advised. Seek urgent care for symptoms such as nausea, vomiting, abdominal pain, rapid breathing, unusual tiredness, fever, severe genital pain or swelling, or signs of allergy. Use Empagliflozin + Linagliptin only under the guidance of a registered doctor or pharmacist.

• Insulin or sulfonylureas can increase the risk of hypoglycaemia; dose adjustment may be needed by a doctor.
• Diuretics may increase dehydration, low blood pressure, and kidney-related risks.
• Strong enzyme or transporter inducers such as rifampicin may reduce linagliptin exposure and effectiveness.
• Alcohol, very low-carbohydrate diets, fasting, or acute illness may increase the risk of ketoacidosis.
• Empagliflozin may affect lithium levels; monitoring may be needed if lithium is used.

• Known serious hypersensitivity to empagliflozin, linagliptin, or any component of the tablet.
• Severe renal impairment, end-stage renal disease, or dialysis, where use is not appropriate.
• Diabetic ketoacidosis or type 1 diabetes mellitus.

Empagliflozin + Linagliptin is generally not recommended during pregnancy, especially in the second and third trimesters, because SGLT2 inhibitors may affect fetal kidney development. It is also not recommended during breastfeeding because of potential risk to the infant. Women who are pregnant, planning pregnancy, or breastfeeding should consult a doctor for safer diabetes management options.

Store tablets at room temperature, away from excess heat, moisture, and direct sunlight. Keep in the original packaging and out of the reach of children. Do not use after the expiry date or if the tablet or packaging appears damaged.
