Dextromethorphan Hydrobromide + Phenylephrine Hydrochloride + Triprolidine Hydrochloride

Dextromethorphan Hydrobromide + Phenylephrine Hydrochloride + Triprolidine Hydrochloride is a cough suppressant, nasal decongestant and antihistamine combination medicine available in Bangladesh under many brand names. Below are its uses, dosage, side effects and precautions — and all brands with their current prices.

• Temporary relief of dry, irritating cough associated with common cold or upper respiratory tract irritation.
• Relief of nasal congestion, runny nose and sneezing associated with common cold, hay fever or other upper respiratory allergies.
• Symptomatic relief only; it does not treat the underlying viral or bacterial cause of illness.

Adults should use Dextromethorphan Hydrobromide + Phenylephrine Hydrochloride + Triprolidine Hydrochloride only as directed on the product label or as prescribed by a registered doctor. The dose and frequency depend on the exact strength of the syrup, age, medical condition and other medicines being taken. Do not exceed the recommended daily dose, and do not use for a persistent or chronic cough unless a doctor advises it.

• Common: drowsiness, dizziness, dry mouth, blurred vision, nausea, stomach upset or constipation.
• Phenylephrine-related effects may include nervousness, restlessness, headache, fast heartbeat or increased blood pressure.
• Triprolidine may cause sedation and impaired alertness, especially with alcohol or other sedating medicines.
• Serious but uncommon: allergic reaction, severe dizziness, confusion, hallucinations, difficulty urinating, irregular heartbeat, severe high blood pressure or breathing difficulty. Seek urgent medical help if these occur.

Use with caution in people with high blood pressure, heart disease, overactive thyroid, diabetes, glaucoma, enlarged prostate or urinary retention, liver disease, chronic bronchitis, emphysema, asthma, persistent cough, or cough with excessive phlegm. It may cause drowsiness; avoid driving, operating machinery or drinking alcohol if affected. Children, older adults and people taking multiple medicines need extra caution. Use under a registered doctor's or pharmacist's guidance.

• MAO inhibitors such as phenelzine, tranylcypromine, isocarboxazid, selegiline or linezolid: risk of severe reactions including very high blood pressure or serotonin-related toxicity; avoid use during treatment and for 14 days after stopping an MAOI.
• Other serotonergic medicines such as SSRIs, SNRIs, tricyclic antidepressants, certain migraine medicines or tramadol may increase the risk of serotonin syndrome with dextromethorphan.
• Alcohol, sleeping tablets, opioids, benzodiazepines, sedating antihistamines or other CNS depressants may increase drowsiness and impaired coordination.
• Other decongestants, stimulants or medicines for blood pressure/heart conditions may increase cardiovascular side effects.
• Do not combine with other cough, cold or allergy products containing dextromethorphan, phenylephrine, pseudoephedrine or antihistamines unless advised by a healthcare professional.

• Known hypersensitivity to dextromethorphan, phenylephrine, triprolidine or any component of the product.
• Use with an MAO inhibitor or within 14 days of stopping an MAO inhibitor.
• Severe or uncontrolled hypertension, severe coronary artery disease or serious heart rhythm problems unless specifically directed by a doctor.
• Do not use for chronic cough due to asthma, smoking, emphysema or cough with large amounts of phlegm unless advised by a doctor.
• Use in very young children should be avoided unless specifically prescribed by a doctor, as cough and cold medicines can cause serious adverse effects.

During pregnancy or breastfeeding, Dextromethorphan Hydrobromide + Phenylephrine Hydrochloride + Triprolidine Hydrochloride should be used only if a registered doctor considers it necessary. Phenylephrine may not be suitable for some pregnant patients because of possible effects on blood vessels, and antihistamines may cause sedation in the mother or infant. Breastfeeding mothers should seek medical advice before use and monitor the baby for unusual sleepiness, irritability or feeding problems.

Store at room temperature, away from excessive heat, direct sunlight and moisture. Keep the bottle tightly closed and out of reach of children. Do not use after the expiry date, and dispose of unused medicine safely.
