What is Xtracal-OT 400?
Xtracal-OT 400 400 mg tablet by Pharmasia Ltd. is a calcium supplement available in Bangladesh. It contains Calcium Orotate and is mainly used to help prevent or treat calcium deficiency and to support bone health when dietary calcium intake is inadequate or a clinician advises supplementation.
Calcium Orotate provides calcium, an essential mineral needed for bones, teeth, muscles, nerves, and normal blood clotting. A key caution is that excessive calcium intake can cause high blood calcium levels or kidney stones, especially in people with kidney disease or a history of stones.
Indications
- Prevention or treatment of calcium deficiency when intake from diet is inadequate.
- Supplementation in people with increased calcium needs, when recommended by a healthcare professional.
- Adjunct support in osteoporosis or osteopenia management, alongside diet, vitamin D, exercise, and prescribed therapy as advised.
Dosage & Administration
Adults should take Calcium Orotate only as directed on the product label or by the prescribing doctor. Calcium supplements are commonly taken with food to improve tolerance. The appropriate dose depends on age, diet, pregnancy or lactation status, kidney function, vitamin D status, and other medicines being used. Do not exceed the recommended dose unless a doctor advises it.
Side Effects
- Common: constipation, bloating, gas, nausea, stomach discomfort.
- Possible with excessive intake: increased thirst, frequent urination, loss of appetite, weakness, confusion, or irregular heartbeat due to high calcium levels.
- Kidney stones may occur in susceptible people.
- Serious but uncommon: allergic reaction such as rash, swelling, dizziness, or breathing difficulty; seek urgent medical help.
Precautions & Warnings
Use caution in people with kidney disease, a history of kidney stones, high blood calcium, high urine calcium, hyperparathyroidism, sarcoidosis, or those taking vitamin D or other calcium-containing products. Do not use multiple calcium supplements together unless advised. Use under the guidance of a registered doctor or pharmacist, especially for long-term use, pregnancy, breastfeeding, children, elderly patients, or people taking regular medicines.
Drug Interactions
- May reduce absorption of tetracycline or quinolone antibiotics; separate dosing as advised by a healthcare professional.
- May reduce absorption of levothyroxine, bisphosphonates, iron, zinc, and some other minerals; spacing doses is usually required.
- Thiazide diuretics and high-dose vitamin D can increase the risk of high calcium levels.
- Use with digoxin or other cardiac glycosides requires medical supervision because abnormal calcium levels may affect heart rhythm.
- Large amounts of bran, phytate-rich foods, or oxalate-rich foods may reduce calcium absorption if taken at the same time.
Contraindications
- Known hypersensitivity to calcium or any component of the product.
- High blood calcium levels or conditions causing hypercalcemia.
- Severe hypercalciuria or recurrent calcium-containing kidney stones unless specifically supervised by a doctor.
- Severe kidney impairment without medical supervision.
Pregnancy & Lactation
Calcium is important during pregnancy and breastfeeding, but supplements should be used only when needed and at doses advised by a doctor or pharmacist. Excessive calcium intake should be avoided because it may cause harm to the mother or baby. Normal recommended supplementation is generally considered compatible with breastfeeding.
Storage Conditions
Store in a cool, dry place away from direct light, heat, and moisture. Keep the container tightly closed and out of reach of children. Do not use after the expiry date or if the tablets appear damaged.
