Pioglitazone helps the body use insulin more effectively by improving insulin sensitivity in muscle, fat, and liver tissues. A key caution is that it can cause fluid retention and may worsen or trigger heart failure, so new swelling, sudden weight gain, or breathing difficulty needs urgent medical advice.
Indications
- Treatment of type 2 diabetes mellitus in adults, as an adjunct to diet and exercise.
- May be used alone or with other antidiabetic medicines when prescribed by a doctor.
- Not indicated for type 1 diabetes or diabetic ketoacidosis.
Dosage & Administration
For adults, Pioglitazone is usually taken once daily, with or without food. Side Effects

Drug Interactions

- Gemfibrozil and other strong CYP2C8 inhibitors: may increase Pioglitazone levels and side effects.
- Rifampicin and other CYP2C8 inducers: may reduce Pioglitazone levels and effectiveness.
- Alcohol may worsen diabetes control and can increase concern for liver-related problems; discuss safe use with a clinician.
Contraindications
- Known hypersensitivity to pioglitazone or any tablet ingredient.
- Type 1 diabetes mellitus or diabetic ketoacidosis.
- Current or severe heart failure; initiation is not recommended in patients with symptomatic heart failure.
- Active liver disease or unexplained persistent liver enzyme elevation.

Pregnancy & Lactation
