What is T4?

T4 (Thyroxine) is a thyroid hormone that regulates metabolism, energy, and growth. It is converted into the more active T3 in the liver and other tissues.

Blood tests measure T4 in two forms: Total T4 (both bound and free) and Free T4 (the biologically active form). Abnormal T4 levels can indicate thyroid dysfunction.

High T4 levels are often linked to hyperthyroidism, such as Graves' disease or thyroiditis. Low T4 levels may suggest hypothyroidism or conditions like Hashimoto’s thyroiditis.

T4 testing is usually done alongside TSH (Thyroid Stimulating Hormone) and T3 to assess overall thyroid health.

What to expect & how to prepare

T4 is performed at a diagnostic centre or hospital lab. A trained technician collects the required sample (commonly a blood draw, urine sample, swab or an imaging scan, depending on the test) and sends it to the lab for analysis. Most routine tests take only a few minutes to sample, with reports typically ready the same day or within 24–48 hours.

• Fasting: some tests require 8–12 hours of fasting (water is usually allowed). Ask the centre when you book.
• Medication: tell the lab about any medicines or supplements you take, as some can affect results.
• Timing: certain hormone and biochemistry tests are best done in the morning — confirm the recommended time.
• Report: always have a registered doctor interpret your T4 result rather than self-diagnosing.
